Im Mittelpunkt des Film stehen zwei Soldaten, Luc Deveraux und Andrew Scott, welche sich gegenseitig während des Vietnamkrieges erschießen, nachdem Scott Amok gelaufen war. 23 Jahre später werden beide im Rahmen des streng geheimen Universal-Soldier-Programms wieder zum Leben erweckt; ihre Körper wurden nach ihrem Tod auf Eis gelegt und nun gentechnisch regeneriert. Eigentlich ohne eigenen Willen, beginnt Deveraux sich an seine Vergangenheit zu erinnern und versucht nun, seinem Dasein als Killermaschine innerhalb des Universal-Soldier-Programms zu entfliehen. Mit Hilfe der Reporterin Veronica Roberts kann er entkommen, wird jedoch von den anderen Soldaten seiner Einheit verfolgt. Schließlich kommt es zum Showdown zwischen Deveraux und Scott, da letzterer sich schließlich auch an die Vergangenheit erinnern kann und eine Rechnung zu begleichen hat.
